1. Determine the extent of the customer's injury and take appropriate action.
2. Address the hazardous situation immediately.
3. Do not assume liability for the incident.
4. Gather names, phone numbers, and statements from witnesses.
5. Fill out the customer incident form found in your claims guidelines folder and fax to Chappell, Smith and Associates, Inc. FAX:
Someone reports a broken tooth or food related illness or injury
1. Seek medical attention as appropriate.
2.
Report the time and date they visited the restaurant.
3. Save any foreign substance that caused the injury.
5. Do not assume liability for the incident.
6. Fill out the customer incident form found in your claims guidelines folder and fax to Chappell, Smith and Associates, Inc. FAX:
For burglary claims
1. Contact the police immediately.
2. Start an inventory of what was stolen.
3. Obtain a copy of the police report.
4. Fill out the property and automobile form below and fax to Chappell, Smith and Associates, Inc.

5. Start compiling supporting documentation of what was stolen (i.e. sales reports and receipts).
For all other property claims
1. If there is structural damage, make the necessary emergency repairs to prevent further damage.
2. Contact the 24 hour claim reporting number at 800-279-7221.
3. You will be asked for your policy number which is on the front page of your claims guidelines folder.
4. We will follow up with you once we are notified of the incident, or you may contact us directly. |
